Preparing for Accreditation Canada’s Qmentum Global™ onsite survey while pursuing BPSO designation provided North Shore Health Network (NSHN) with an opportunity to align quality improvement priorities and accelerate evidence-informed practice change. Rather than treating accreditation and BPSO work as separate initiatives, an interprofessional team of leaders, clinical staff, allied health professionals, and quality specialists integrated these efforts to advance organizational readiness.
Key areas of focus involved aligning the LTC and Service Excellence Qmentum GlobalTM Standards with implementation of RNAO’s BPGs for:
- Person- and Family-Centred Care
- Care Transitions
- Preventing Falls and Reducing Injury from Falls
This process identified multiple areas where BPG recommendations directly supported Qmentum Global™ criteria and Required Organizational Practices (ROPs). One example was the redesign of the care conference process, which strengthened interdisciplinary collaboration, enhanced resident and family engagement, and supported more person-centred care planning and transitions.
Guided by RNAO’s Knowledge-to-Action (KTA) Framework, Social Movement Action (SMA) Framework, and Leading Change Toolkit, our interdisciplinary team used the BPG Gap Analysis Tool and Action Plan to assess current practices, identify priority gaps, engage stakeholders, and implement targeted improvements. The KTA Framework provided a structured approach to implementing, evaluating, and sustaining change, while the Leading Change Toolkit and SMA Framework supported readiness assessment, staff engagement, champion development, communication, and shared ownership of improvement initiatives.
Implementation outcomes demonstrated strengthened care planning processes, improved alignment with Qmentum Global™ criteria and ROPs, enhanced resident and family engagement, and increased evidence of quality monitoring and continuous improvement activities.
Lessons learned highlighted the natural alignment between BPSO designation and Accreditation readiness. Integrating these initiatives reduced duplication, improved efficiency, and created a sustainable framework for advancing organizational priorities. This presentation will share practical strategies for leveraging RNAO implementation frameworks and tools to strengthen Accreditation readiness while embedding evidence-based practice into everyday care.
