Description

Moving from organizational readiness to sustained impact, Long-Term Care homes must solve the critical challenge of "champion fatigue". Our Best Practice Spotlight Organization (BPSO) recognized that clinical guidelines succeed only when front-line staff leading them are energized, resilient, and aligned with their roles. True clinical sustainability requires a shift from passive capacity-building to active, person-centered engagement that acknowledges workforce pressures.
Initiative Description
The purpose of this initiative was to revitalize our initial champion network by transitioning from generic, task-oriented roles to a data-informed "Change Power" framework, thereby increasing interprofessional engagement and accelerating clinical pathway adherence.
Impact and Outcomes
Grounded in the Knowledge-to-Action (KTA) framework, we conducted a mixed-methods baseline assessment mapping the professional strengths, individual preferences, and burnout levels of 45 new and veteran champions. Moving away from generic assignments, we strategically integrated champions into clinical program teams matching their mapped strengths. We established bidirectional communication loops during clinical pathway rollouts and utilized equity-focused coaching models to ensure diverse staff representation across all shifts and designations.
Results / Outcomes
Post-implementation data revealed a 6% increase in active champions, with over 25% increase in participation rates at program meetings and a significant rise in the frequency of interprofessional information exchange. By aligning individual "change power" with operational needs through the COACHES specialized education framework, the home achieved stronger, more consistent adherence to new clinical pathway timelines. the home to new clinical pathway timelines. Lessons Learned & Implications for Practice
Meaningful change is driven by people and sustained through adaptive practice. Revitalizing a champion network requires moving past static volunteer lists toward personalized, strength-based deployment. This approach provides an scalable roadmap for other BPSOs to turn implementation capacity into a permanent cultural standard of clinical excellence.
