At Copernicus Lodge, our mission is to provide the highest quality of holistic, resident-focused care grounded in dignity, respect, compassion, and partnership. We selected the Person and Family Centred Care (PFCC) Best Practice Guideline to strengthen our commitment to placing residents and families at the centre of care. Baseline assessments identified opportunities to improve the admission experience, strengthen understanding of residents’ life histories and preferences, and build staff confidence in engaging in meaningful care conversations.
The aim of this initiative was to implement the PFCC Best Practice Guideline to enhance the resident move-in experience and embed person- and family-centred principles from the earliest stage of the resident journey. The initiative sought to strengthen resident and family partnerships, improve interdisciplinary collaboration, support meaningful care conversations, and ensure care planning reflects individual preferences, values, and goals.
To address identified gaps, we implemented standardized person- and family-centred strategies to improve admission and care planning processes. These included consistent use of residents’ preferred names, enhanced care plans reflecting preferences, routines, life histories, and cultural considerations, and adoption of PFCC-aligned language. Prevention of Error-Based Transfer (PoET) was introduced to support meaningful goals-of-care conversations. Staff education, coaching, and interdisciplinary engagement were provided, with active involvement of residents and families in care planning and decision-making.
Implementation resulted in positive changes in culture, processes, and daily practice. Staff demonstrated increased awareness and integration of PFCC principles. Residents reported greater comfort expressing preferences and participating in care decisions, while families experienced improved communication, inclusion, and satisfaction.
Meaningful practice change requires time, sustained effort, and staff engagement. Success depended on staff buy-in, ongoing education, and confidence building. Sustaining improvements requires continuous monitoring, reinforcement, and knowledge translation. These findings highlight that lasting change depends on leadership support, open communication, and a shared commitment to embedding evidence-based PFCC into everyday practice.
