Project Overview
The Legacy Project is a person-centered storytelling initiative for residents in long-term care. Residents create meaningful keepsakes using stories, letters, poems, reflections, and photographs that celebrate their lives and memories.

Purpose
To honor residents’ lives, strengthen relationships with families and staff, and support emotional well-being through meaningful engagement.

• Promotes self-esteem and emotional wellness
• Encourages cognitive engagement
• Builds stronger resident, family, and staff relationships
• Supports person-centered care
• Creates lasting keepsakes for loved ones

How It Works
Residents and families participate in guided interviews or complete legacy templates. Staff help collect stories, photographs, and reflections that are assembled into a scrapbook or memory book.

Vision
To ensure residents feel valued, remembered, connected, and supported throughout their journey in long-term care.
