Description

Completing a gap analysis while implementing a Best Practice Guideline (BPG) is an opportunity to recognize what an organization is doing well and identify areas for improvement. The first step in the Knowledge-to-Action framework is to clearly define the problem and the gap. To ensure these gaps reflect real practice, engaging direct care clinicians is essential.
Traditional group discussions can limit participation. Feedback from clinicians shows that some are uncomfortable speaking publicly, others dominate conversations, and differing perspectives can make it hard for all voices to be heard.
To address this, our organization used an audience response system to support a more inclusive and effective gap analysis. We developed structured questions aligned with each Good Practice Statement from the Transitions to Care and Services BPG. These included ranking questions (e.g., how well we perform certain interventions, and which should be prioritized) as well as open-ended questions for more in-depth feedback. Responses were submitted anonymously, allowing everyone to contribute freely, and then discussions were held based on the responses.
This approach was used with our Nursing Practice Council, which includes nurses from inpatient and outpatient settings, as well as various leadership levels. To maintain engagement, we incorporated short trivia “brain breaks” between sections.
Participation averaged 90%, and the process identified two key areas for improvement. Participants also shared that this format was more engaging, educational, and inclusive compared to traditional open discussions, where not everyone typically contributes.
Overall, this method created a safe, structured space for meaningful input and will continue to be used as we implement future BPGs.
