Background: The RNAO People-Centred Care (PCC) Best Practice Guideline (BPG) provides a strong foundation for advancing PCC across healthcare sectors. However, many organizations face challenges translating these principles into consistent, measurable practice. At VHA Home HealthCare, a comprehensive gap analysis highlighted the need for a more practical and accountable approach to operationalizing PCC.
Purpose: This presentation describes two change ideas that shift PCC from an abstract concept into tangible, observable actions. We outline the co-design process and early impacts of improvements to VHA’s care planning process and the development of a Client and Family Declaration of Values (DoV).
Implementation Approach: Using a co-design approach, VHA partnered with client and family partners, staff, service providers, and leadership to enhance care planning and develop a DoV grounded in lived experience. For care planning, focus groups and consultations ensured changes reflected the needs of clinicians, clients, and families. The DoV was developed through a rigorous process including value validation, surveys, focus groups, and iterative feedback to ensure alignment with both evidence and lived experience.
Results: These initiatives demonstrate how PCC can be operationalized at different levels of the organization. Care planning improvements enhanced clarity and consistency for clients receiving personal support services, with evaluation guided by RNAO PCC BPG indicators. The DoV operationalizes PCC more broadly, defining values in plain language with specific, observable behaviors. Early evaluation focuses on accessibility and awareness.
Lessons Learned: The collaborative approach for care planning and DoV development offers a scalable strategy to bridge the gap between PCC principles and practice. The work demonstrates how RNAO’s PCC BPG can be effectively operationalized through meaningful client and service provider engagement.
