Description

Villa Colombo Toronto, a Best Practice Spotlight Organization designated in 2022,expanded within the OHT(2024), has implemented a comprehensive, technology-enabled model to embed,adapt& sustain evidence-based practices across LTC. This initiative integrates clinical pathways derived from RNAO BPGs with advanced digital infrastructure to enhance quality, safety & person family-centred care.

At the core is a coordinated medication management system leveraging PointClickCare, Medisystem pharmacy integration,& Taper-MD quarterly medication review platform. This closed-loop process includes pharmacist-led order entry, nursing verification checks, automated communication through Engage⁺,& secure interdisciplinary collaboration within PCC. Innovations such as pharmacy-led medication reconciliation pilot, stat dispensing technology & digital transcription tools i.e Digipens have strengthened medication safety & reduced risk.

Clinical pathways aligned with BPGs—such as falls prevention, pain management, delirium/dementia care, palliative care,& EOL —are operationalized through standardized workflows & supported by technologies like Marlann for workflow automation & PREVIEW-ED for early detection of clinical deterioration. Additional innovations include AI-driven compliance monitoring (TruNorth Navigation),real-time benchmarking (OnSpark),& integrated hospital data access (Project AMPLIFI,e-Connect) enabling proactive,data-informed decision-making.

Family engagement is enhanced through the Engage⁺ portal,providing real-time updates,secure communication,& streamlined consent & documentation processes aligned with legislative requirements (FLTCA, PHIPA). Equity and accessibility are addressed through inclusive communication strategies, culturally responsive care (serving a diverse, predominantly Italian-Canadian population),&expanded access to on-site services such as palliative care rounds, IV therapy, & specialty clinics.

Evaluation is driven by continuous quality improvement using real-time data, including falls reduction initiatives (Momo/Ponto sensors), wound care advancements(NanoSalv),& infection prevention innovations (GeneXpert). Outcomes demonstrate improved early intervention, reduced ER transfers, enhanced interdisciplinary collaboration,& increased staff engagement through mentorship & champion programs.

Key lessons highlight the importance of integrated digital ecosystems, interdisciplinary collaboration,& adaptive implementation strategies to sustain evidence-based practice. This model offers a scalable framework for LTC homes seeking to align innovation with BPGs while advancing quality, compliance,& resident-centred outcomes.
